PRESCRIPTION REFILLS: We DO NOT refill prescriptions by email request, we ONLY refill prescriptions IN THE OFFICE,
Please plan ahead so make an appointment when you have 2 weeks left of your medications.
WE DO NOT PRESCRIBE NARCOTICS or OTHER CONTROLLED DRUGS.
We do NOT give medical, dental or mental health advice via email.
If you do NOT provide a correct email, we will have NO WAY to make or inform you of an appointment.

IF SPRINGFIELD SCHOOLS ARE OPEN: LIFT UP WILL BE OPEN.

LIFT UP WILL OPERATE ON A SLIDING INCOME FEE BASIS FOR ALL PATIENTS IN ORDER TO BE FAIR TO ALL.
PLEASE SEE ELIGIBILITY PAGE FOR WHAT IS REQUIRED OF ALL NEW AND EXISTING PATIENTS TO ESTABLISH YOUR LEVEL OF PAY.
If you haven't yet established your level of pay, you will be required to sumbit one of the 4 documents listed on the
eligibilty page to be seen. If you do NOT have any of the required documents, you will automatically be listed on
the top tier until you are able to establish your level of eligibility. Please bring your form with you at the time of
your appointment to establish your level for the year. THANK YOU
